1 In the case of the existence of a liquidation note, the request is canceled and a copy of the liquidation note signed by the president and a copy of the transfer
request is specified to be nulled and is signed by a member from the office of the cult from which ititialy it has been transferred the person.
2 It is completed in the case of the Mosaic Cult if the person did not pay the cult until 31 December, in the year before this application, and it is not necessary if
the person of Jewish origin has not paid for the last two years, but is necessary for people who do not know they have Jewish origin or genalogical tree.
